1．稱謂：仲裁中當事人的標準稱謂為申請人、被申請人；
2．名稱：（1）當事人名稱須與合同中名稱及所蓋公章名稱一致。否則，須提交變更證明或書面說明等材料（數量同提交申請書份數）
（2）合同中一方為外方且使用英文名稱，則申請書中應以該英文名稱提起仲裁；
3．地址：須為能夠有效送達的地址、郵編；
